What Happens in Kaysville, and What Does Not
You can book the first visit in the office, or by video if you are in Utah, at the same price. Your labs are drawn at a LabCorp location before the visit, so your provider has results in hand on the day.
The 3D body composition scan happens only in the office. It comes with the Weight Loss Assessment and the Comprehensive Health Assessment; a first visit booked by video skips it.
After that, check-ins are in office, by phone, or by video while you are in Utah, and questions go to your provider in the app. Labs are rechecked every three or six months, depending on your plan.

FAQ
Common Questions
No. Our one clinic is at 186 N 100 E in Kaysville, about eight minutes from Layton’s city center by Fort Lane, ten to twelve from the far north and west by I-15. You can book the first visit there, or by video if you are in Utah; the scan happens only in the office.
From central Layton, Fort Lane: it runs south to Kaysville’s Main Street with no freeway. From west Layton and Hill Field Road, I-15 south to Exit 328, 200 North, is quicker. I-15 southbound slows in the morning, and northbound jams from three to six on your way home, so Fort Lane earns its keep at rush hour.
If you are a civilian or a contractor, no. From the South Gate it is Hill Field Road to I-15, then two exits south, about eleven minutes. If you are active duty, talk with your base provider first: we are a cash-pay clinic outside TRICARE.
Mostly, yes. Between visits you message your provider in the app, request refills and see your lab results. Check-ins can be in office, by phone, or by video while you are in Utah. Labs are drawn at a LabCorp location and rechecked every three or six months, depending on your plan. If you will be away for a long stretch, tell your provider ahead of time.
No. We do not bill TRICARE or any insurance, so care with us is a separate choice you pay for, not a benefit. Retirees, spouses and civilians book directly. Labs can go through our LabCorp cash price or your insurance. Many members use their HSA for care at MTN HLTH. We recommend checking with your plan about what's covered, and we're happy to provide any documentation you need to submit, including the services provided and their purpose.
Yes. Syracuse and the other towns around Hill share this page, because the trip to Kaysville is the same southbound drive. From south Syracuse, the West Davis Corridor runs to 200 North in Kaysville, about seventeen minutes. From Syracuse’s city center, Antelope Drive to I-15 takes about fifteen.
